• Regular checks of your blood pressure, pulse and weight between visits to your doctor.
• Periodic checks of your blood sugar level.
• A review of your medications, discussion of your health concerns, and referrals
for you, as needed, to services offered by other agencies.
• Educational programs and materials on health topics important to you!
