----- FREE COMPLETE BLOOD SCREENINGS & BLOOD PRESSURE CHECK

Screenings include:
(1) Total Cholesterol (2) HDL (good cholesterol) (3) LDL (bad cholesterol)
(4) Coronary heart disease risk factor (5) triglycerides (6) cholesterol/HDL ratio
(7) LDL/HDL ratio (8) VLDL

Also Available: PSA Prostate Test • TSH Thyroid Test • Hemoglobin A1c Diabetic Test
(free with Doctor's Order - Fax order to Sue Eden at 419-774-0257)

All tests remain confidential and are sent to the individual with a detailed explanation of the results. Individuals with abnormal test results will receive a form letter suggesting repeat testing and follow-up with a physician. A copy of the results may also be forwarded to the individual's primary care physician if desired.

FOR ACCURATE RESULTS, DO NOT EAT OR DRINK ANYTHING FOR 12 HOURS! (water & medications are okay). The American Heart Associations recommends NO ALCOHOL for 24 hours for test accuracy.

Available to:
• Employees and Spouses enrolled in the county insurance plan. MUST BRING INSURANCE CARD.
• Empoyees and Spouses enrolled in the buyout option.

• Employees NOT enrolled in the county insurance plan pay $27.00. PSA, TSH, and Hemoglobin A1c are an additional cost.

Employees may schedule their appointment at any county agency throughout the year (see schedule below).
